For new transfer in student from other school, you need to submit the following"

1. Xerox PSA Birth Certificate

2. Certificate of Good Morale

3. Form 138

4. Form 137 (SF10)

5, Certificate of Awards (If there are)

For K1  Docs Required

1. Xerox PSA Birth certificate only
